Steve Chafe, ATP, MIE
Steve Chafe has over 18 years
of military experience and currently serves in the U.S. Air Force Reserve as a
C-130 Instructor Pilot, Assistant Director of Operations, and he facilitates the
Crew Resource Management program for the 440th Airlift Wing. He has brought this
same intense CRM training to over 700 physicians, nurses, and other health
professionals at hospitals throughout the country including The University of
Missouri, Provena Health System, University of California—Irvine, and the Mount
Carmel Health System. In addition, he is also qualified to conduct Risk
Assessments and Hardwired Safety Tools workshops.
Mr. Chafe's flying experience includes more than 400 combat sorties in Iraq,
Afghanistan, and the Horn of Africa. He was hired by American Airlines in 2001
and is qualified in the MD-80 aircraft.
Steve Chafe received a Bachelor's Degree in Aeronautical Engineering and was
commissioned into the Air Force from Embry-Riddle Aeronautical University in
Florida. Mr. Chafe was awarded a scholarship from the Air Force Institute of
Technology to pursue a Master's Degree in Industrial Engineering with a focus on
process improvement and human factors. He then was assigned to the Air Education
and Training Command as a Flight Test Program Manager and Test Director.
