Since
the Institute of Medicine report,
TO ERR IS HUMAN: BUILDING A SAFER HEALTH
SYSTEM, which estimated the number of deaths in the U.S.
each year due to preventable medical errors to be between 44,000 and
98,000 people, LifeWings has been focused on providing services

defines a medical error to be "...the failure of a planned action to
