The Australian Patient Safety Foundation (APSF)

APSF is an
independent, incorporated, non-profit organisation. It has been engaged in the
process of finding problems and proposing and developing solutions for patient
safety for 18 years. The APSF has established systems to collate and analyse
information relating to patient safety issues. From this base, the APSF is able
to support the development of strategies to better detect, manage and avoid
preventable harm and to make a better and safer place for patients and residents
of health care and residential care facilities. These solutions can include
technical (developing algorithms for standardised practice), organisational
(embedding tools such as incident management systems), cultural (changing norms
to encourage reporting of problems) and regulatory solutions. Our core purpose
is the elimination of preventable harm in the health care environment and our
philosophy is
to continuously seek better ways of achieving this aim. Apart from the work in
analysis and research, the APSF has developed (and commercialized) an incident
reporting system called AIMS (www.patientsafetyint.com)
which gives healthcare organisations the ability to capture their incident
information in a meaningful way.

Healthcare Quality (NICHQ)

The National Initiative for Children's Healthcare Quality (NICHQ) is an
action-oriented, not-for-profit organization dedicated solely to improving the
quality of health care provided to children. Founded in 1999, NICHQ's mission is
to eliminate the gap between what is and what can be in health care for all
children. A national organization with its home office in Cambridge, MA, NICHQ
works with staff and faculty across the country. The main topics of this site
are improvement methods and patient safety for children.
